I am concerned with the disregard for patient privacy. There is no patient intake form, instead the staff asks for your information while at the counter like giving your SSN to them, out loud, in a waiting room with other patients. This was then scribbled onto scratch paper with other patients' information. They didn't ask about my ethnicity, but simply guessed & got it wrong.
