Staff was unprofessional - wanted me to sign blank piece of paper that they claimed they'd later print a form on, never telling me what the form was or what i would be agreeing to. totally unprofessional, unethical, and sketchy. took months to get a appointment with this dr., he personally set up appointment so i advised at the time of my reason for visit, yet at time of visit he did nothing,...
