Horrible service there with the staff. Called them to advise them that our insurance changed and was told that I have to get on my company letterhead that I am really insured with the policy & group #. Then she went on that I also have to get the address of Horizon where they have to bill to. I found out none of this is true so I cancelled the appointment. I waited for 5 weeks for lab results...
